Enlarged breast tissue in men, known as gynecomastia, is far more common than most men think and far more distressing than most people realise. It changes what you wear, whether you swim, and how you stand. When it has persisted past puberty, or appeared in adulthood and not responded to weight loss, surgery is the reliable way to flatten the chest.

Fat, gland, or both
The right technique depends on what the enlargement is made of, which is why examination comes before any plan.
- Fatty tissue (pseudogynecomastia). Soft, diffuse fullness that responds to liposuction of the chest alone.
- Glandular tissue (true gynecomastia). A firm disc behind the nipple that liposuction cannot remove. It is excised directly through a short incision along the lower edge of the areola, where the scar is hard to see.
- Both. The most common finding, treated with liposuction to shape the chest and excision to remove the gland in the same session.
Where there is a lot of loose skin after major weight loss, a skin-reduction technique is discussed separately; it leaves longer scars and is not the routine approach.
Before surgery is recommended
Gynecomastia can be linked to weight, hormones, certain medicines and supplements, or have no clear cause. We take a proper history and, where indicated, ask for blood tests, because a treatable cause should be corrected first. Surgery is for tissue that persists once that has been ruled out. Patients close to a stable weight get the most predictable result.
How the procedure is performed
- Assessment and marking. The chest is examined standing and lying down to judge how much is fat and how much is gland, and the areas are marked with you.
- Anaesthesia. General anaesthesia in most cases; local anaesthesia with sedation is possible for liposuction-only treatment.
- Liposuction. Through incisions of a few millimetres at the side of the chest, fine cannulas remove the fat and blend the contour into the surrounding chest.
- Gland removal. Where a gland is present, it is removed through a short incision at the edge of the areola and the area is closed with fine sutures.
- Compression vest. Fitted immediately and worn for about 4 weeks to help the skin settle flat. The whole procedure takes 1 to 2 hours, as a day case or with one night in the clinic.
Recovery and results
The chest feels bruised and tight for about a week. Desk work is usually possible after 3 to 5 days, flying home after 5 to 7 days, and chest exercise after 4 to 6 weeks. The flatter contour is visible early and refines over 3 months as swelling settles. Removed gland does not grow back; fat can return with significant weight gain, so a stable weight protects the result.
